If you would like to make a donation, please email this donation form to the Office of the College of Life Sciences, National Yang Ming Chiao Tung University |

If the donor provides the ID number, the donation information will be uploaded to the National Taxation Bureau and the donor can download the donation information from the National Taxation Bureau when filing the income report in the following year without attaching the donation receipt. |
